Cancellation form
This form may be used when the consumer wishes to cancel an agreement, in accordance with the provisions of the Distance Contracts Act and the Act on Contracts Concluded Outside Business Premises (SFS 2005:59). The consumer also has the option to cancel in another way than by using the form, but is always recommended to keep documentation showing that he or she has cancelled.

The form must be completed and returned only if you wish to cancel the agreement. Please note that it must be sent to the relevant company, not to the Swedish Consumer Agency.
